Personalisation and Engraving of Medical Tags
This page details the engraving options available on our medical tags and watches. If you have any queries or special requirements please do not hesitate to call us on 0121 233 7455.
We offer FREE engraving of up to 6 lines of details. Above is a picture of the standard layout for medical tags. Regrettably we are unable to engrave the SOS Talisman, Allertag Capsule or ZapTag range of products. Engraving on Nurses Fob watches is at extra cost. Please call for details.
When the tag is ordered please type into the "Information" box the information that you want to be engraved on the back of the tag. Don"t worry about the layout of the tag as we will decide on the best way of engraving.
Name - Your own name
Date of Birth - Your date of birth dd/mm/yyyy(often used by medical establishments to assertain your identity)
Contact No. - This is a phone number that you would like to be called if you are taken ill, usually a relative or friend.
If you are ordering a Medical donorcard tag - ideally the Contact No. is for a relative or Next of Kin.
If you wish to add the international dialing code for the UK (+44) case you are taken ill abroad, please state this in the order form.
Doctors No. - This is your doctors general surgery phone number. If the number is called out of hours a recorded message gives an emergency number to call for your health records. If however, you have been given a specific emergency number to call, please use this for the engraving.
Ailments - Your critical ailments/allergies that would be required by medical staff should you be taken ill.
*** You MUST state the word "NO" or "ALLERGY" when listing an allergy. Stating simply "PENICILLIN" will cause confusion as to whether you are taking penicillin or must not be given the drug ***
How to order
Tags can be ordered in three ways
- by phoning 0121 233 7455. We will take your order and payment details and process them accordingly
or
- by clicking on the "Buy" button near to the item you want to purchase. After approximately 25 seconds you will be forwarded to an order page. This will show what you are about to order. If you wish you can press the "ORDER VIA SHOPCREATOR" button. You will then be asked for your address and credit card payment details. Beneath this is a box entitled "Engraving". Here you can type in the details you want on the back of the tag.

Once you have successfully submitted the order you will receive an email confirmation. Your tag will arrive approximately 2 to 4 days later. Or, if you are unsure about anything, you can press the "E-MAIL ENQUIRY" button. We will then contact you as soon as we can to discuss your requirements.
